How it Works

Acetazolamide works predominantly by inhibiting the carbonic anhydrase II enzyme located in the renal tubular cells of the kidneys. With less CO2 and H+ ions being reabsorbed back into the blood, more is excreted in urine. This results in:

- Metabolic acidosis - Lowering of blood bicarbonate levels due to renal bicarbonate wasting.

- Hypocapnia - Decreased CO2 levels in the blood and body fluids like cerebrospinal fluid (CSF).

- Diuresis - Increased electrolyte-free water loss leading to frequent urination.

This action profile makes it useful for conditions where inducing diuresis, reducing CSF pressure, or altering acid-base balance is beneficial.

Uses of Acetazolamide

Glaucoma
As a diuretic, acetazolamide reduces fluid pressure inside the eye and is used for the initial treatment and prevention of acute attacks in acute/chronic angle closure glaucoma. The drug lowers intraocular pressure within hours.

Altitude Sickness
By facilitating acclimatization to high altitudes, Acetazolamide prevents and treats symptoms of acute mountain sickness like headache, nausea, fatigue, and dizziness. It does so by improving ventilation and acclimatization to low oxygen conditions at high elevations.

Epilepsy
The drug helps control seizures as it lowers CSF pressure and volume. It may be used as an adjunct treatment in certain refractory epilepsy cases.

Cerebral Edema
Acetazolamide's diuretic effects help decrease brain swelling associated with conditions like brain tumors or head injuries.

Heart Failure
Its carbonic anhydrase inhibiting action aids management of peripheral edema in congestive cardiac failure by promoting sodium excretion and fluid removal. This improves symptoms.

Porphyria
Acetazolamide is useful for intermittent attacks of acute intermittent porphyria by lowering ALA and porphobilinogen levels which build up during attacks.

Kidney Stones
The drug increases urine pH and volume, making it useful as a preventive measure for recurring kidney stone formation, especially calcium oxalate stones.

Side Effects and Precautions

While generally well tolerated in most people, some common side effects of acetazolamide use include:

- Tingling of fingers and toes (paresthesia)
- Fatigue, weakness
- Nausea, vomiting, diarrhea
- Loss of appetite
- Crystal formation may rarely occur in urine

It should be used cautiously in people with hepatic or renal impairment as the drug is excreted through these routes. Electrolyte disturbances are also a possibility. Women who are pregnant should avoid acetazolamide unless absolutely necessary due to the potential for harming the fetus.
